What is Advance Care Planning Coaching?

Advance care planning is a conversation about the medical care you want throughout your life and at the end-of-life. It can also be scary to talk about – you don’t know what the future holds, and you don’t want to burden your family.

We hear you! Coaching can transform these hard conversations into ones that’ll give you and your loved ones peace of mind. You’ll get the medical care you want and your family won’t need to guess.

“Caitlin helped me figure out what I want in regard to my health,  helped me talk about this with my wife, and also helped us figure out how to care for me as I get older. Now, we have a designated healthcare proxy and one less thing on our to-do list.”

New England

“Even though I am (relatively) young and healthy, it’s important for me to have my affairs in order to have one less thing to worry about.

Not only did Lisa help me clarify my own values and what was important to me should I be unable to communicate health care decisions, but our session was a wake-up call as it brought out other wishes that I wanted to work on *now*, including the legacy that I want to leave for my children and wife.

“After the coaching session, I felt well-informed and was able to stay focused on my immediate, short-term, and long-term needs.”

How Does It Work?

1. You’ll meet your coach virtually, from the comfort of your home.

You’ll meet with one of our coaches who have over 55+ years of combined experience  with advance care planning.

2. During the coaching session, we’ll help you:

Define what’s most important to you – your values, goals, and wishes for your medical care.

Personalize your advance care plan and create a framework for making healthcare decisions.

Identify and document a Health Care Proxy

Talk with your loved ones and your medical team about your wishes.

Get emotional and spiritual support because it can be scary to think about the future.

3. After the session, you’ll also get…

A personalized action plan with your wishes written out!

Keep a copy for yourself and share a copy with your medical team, and your family. We’ll also collaborate with your primary care clinician to keep them in the loop.
